State Tax Exempt CertificateCustomers in Utah
who wish to remain tax exempt must submit a valid
tax exempt certificate with ISI. You can find
it at the above Utah State Government website in
Adobe Acrobat format. Note that
valid certification numbers must be obtained from
your respective Department of Revenue/State Tax
Commission office.
Request for Taxpayer Identification Number and
Certification (Form W-9)
Customers who need to
submit Form W-9 can find it at the above IRS website
